Online psychotherapy involves more than a video call link. Suitability, privacy, technical conditions and an emergency plan are considered together in light of the client's circumstances.
The client's needs, level of risk, location and ability to attend sessions regularly are assessed. Referral to alternative services may be made where the online format is insufficient or in-person support would be safer.
A space is planned where others cannot overhear the session, the client can speak freely and, where possible, there are no interruptions for its duration. In shared living spaces, arrangements such as using headphones, placing a notice on the door and setting aside session time in advance can be discussed.
The connection, camera, microphone, device battery and platform are checked before the session. It is agreed in advance how long to wait if the connection drops, which secure channel will be used to reconnect and how the session will continue.
Topics such as not sharing session links, notification and account security on shared devices, document transfer and not recording sessions are discussed openly. Only necessary information is used, and the technological limits of confidentiality form part of informed consent.
The client's location during the session, an emergency contact and local healthcare resources are clarified as needed. It is explained that online psychotherapy is not an emergency crisis line and that immediate risk requires contacting local emergency services.
Expectations are clarified around not joining a session while driving or in a public place, setting aside time and attention for the session, communication channels between sessions and response times. The aim is to maintain the therapeutic framework and regular follow-up in the online format as well.
Preparing conditions in which you can be heard and express yourself comfortably, without having to create a perfect setting.
